Care Before Dialysis

Dialysis is a non-phsiological situation, patients will feel psychological imbalance on this stage, one is they feel condition deteriorated, second is the fear of dialysis itself, they maybe lose confidence. so mental nursing is more important on this stage. We should encourage patients to overcome this psychological anxiety and pain, also explain the principles and effects of hemodialysis to paitents and their families completely, and we can invite the patient who has long treatment experience to explain to the new dialysis patient ?so that to help them. While dialysis can maintain the lives of patients, but they will increasingly aware of their physical condition after a long-term treatment, crisis maybe come because of many other reasons, so the nurse should be the good bearer of patients’ anxiety, anger and fear, and also be the supporter for patients and their families at that time.

Nurse must be familiar with characteristics of each patient, make different nursing care plan (NCP). Patients’ diet, dialysis plan and medication shoule be formed a related entirety. Change the dialysis plan if diet changed. Measure patient’s weight, pulse, blood pressure, body temperature and breath before each dialysis. Blood check K+, Na+, Cl-, blood urea nitrogen (BUN), serum creatinine, CO2CP, prothrombin activity, hemoglobin, etc. to understand heart, lung, liver, kidney function and anemia, infection, bleeding and cruor condition. As an example, patients need transfusion when hemoglobin < 50 g/L, infuse dextran by vein or transfusion when low blood pressure, then doing hemodialysis after correcting low blood pressure, and check all the parts before dialysis.
